TTN Caring Collaborative is pleased to announce a partnership with Visiting Nurse Service of New York, including an ongoing series of Health Strategies Workshops and special home care services and benefits for TTN-CC members. 

VNSNY provides TTN-CC members with information and resources to further the goal of healthy independence. These include: 

  • www.VNSNY.org/answers: a user friendly on-line algorithm featuring various medical issues and concerns and the types of home care services applicable to each situation; 
     
  • Discounts on private pay home care services through its affiliate, Partners in Care.  TTN members are eligible for a 10% discount off the first week of private pay services (after complete 1st week).  Services available include skilled nursing care, bath service and case management.  TTN members must call 1-888-GET-HELP (1-888-943-8435) and send in printed coupon with first invoice to receive offer*.
     
  • MD Referral Hotline where you can access help with discharge planning options in the hospital and work with your physician in the community for required referrals.  VNS has staff in 57 hospitals throughout NYC and the hotline can help you access on-site information and support when you are hospitalized.  TTN members can call (866)-632-2557 and must identify themselves as a TTN member.

Home Care 101: Premiere Health Strategies Seminar Received Enthusiastically by TTN-CC Members
 

Home Care 101 was the first topic in our planned Health Strategies Series. Thirty TTN members enthusiastically turned out on a rainy Thursday night in June to find out how they can stay healthy and independent in the community through an exciting educational initiative launched by the Caring Collaborative and the Visiting Nurse Service of New York City (VNSNY). Clinical Director Michael Soccio, RN, MS discussed home care services and presented various scenarios to illustrate eligibility and types of help available. He also described special benefits that VNS has made available to TTN members. 

Participants at the first Health Strategies seminar heard TTN member and physician Diana Killip, MD describe her own observations about growing older and being “meta-stable” or like a “penny on edge.”  “You do a pretty good job of keeping everything together - you’re stable– but also know that any insult can put you down flat.”  Diana stressed that there’s a major role for delaying frailty and disability and offered practical tips that included: take extra care when you get a new pair of glasses; think about how you shave your legs – are you standing up, sitting down, how is your balance; avoid injuries including too much sun exposure; think 10 years out whenever you’re making modifications to your living environment – install good lighting, grab bars in the shower and hallway; and when accidents or illness happen, get over it quickly.  “The goal is healthy independence, not stupid refusal to ask for help,” she said.
