The Caring Collaborative is open to all New York City members of the Transition Network.. Joining is easy; just follow the steps outlined below and you’re in!
 

• Complete the Caring Collaborative online questionnaire by completing a survey. The questionnaire includes three sections:  demographic information one must complete for full participation; information about the services you are willing to provide; and information you are willing to exchange. It takes fewer than 10 minutes to complete.
   
• Attend a Caring Collaborative orientation for a fun, and informative introduction to the program. Orientations are without charge and you can register for the next orientation by checking our calendar for the next scheduled orientation. 
   
• Read and Sign the Caring Collaborative Member Agreement form. Forms are offered at the close of each orientation. Please note that this form must be on file in order to activate your membership in the Caring Collaborative.  

 The Caring Collaborative is currently open to all New York City members.
